RankOne Online Forms Instructions
Things you’ll need:
-smartphone/tablet/computer with a mouse or touchscreen
-parent and student must sign the forms at the same time
If the student is NEW TO OUR SCHOOL DISTRICT THIS YEAR, there will NOT be a profile for them and you must talk to the Athletic Trainer about creating a profile first!
1. Monahans ISD Rank One– IT MUST *NOT* SAY “FINE ARTS”
2. SELECT: “PROCEED TO ONLINE FORMS”
3. SELECT: “CONTINUE AS GUEST” OR YOU MAY CREATE AN ACCOUNT AND FOLLOW THE PROMPTS (IT IS NOT NECESSARY TO CREATE AN ACCOUNT, BUT YOU MAY IF YOU WOULD LIKE)
4. ENTER THE STUDENT’S NAME, DATE OF BIRTH, GRADE FOR 23-24 SCHOOL YEAR, AND SELECT THE CAMPUS THEY WILL BE ATTENDING.
a. IF THEY HAVE 2 LAST NAMES LISTED ON THEIR SCHOOL DISTRICT FORMS, THEY NEED TO USE BOTH NAMES ON THE RANKONE FORMS (i.e. FERNANDEZ GARCIA or LOZANO-HERNANDEZ)
b. DIGITALLY “SIGN” THE FORM, THEN SELECT “SUBMIT”
5. SELECT: “START FORMS”
6. CLICK ON ONE OF THE FORMS IN BLUE TEXT AND FILL IT OUT COMPLETELY. SOME FORMS MAY HAVE CHECK BOXES WITH A HYPERLINK WHICH ALLOW YOU TO VIEW THE FORMS BEFORE CHECKING THE BOX. SIGN THE FORM AND CLICK “SUBMIT” AT THE BOTTOM
7. SELECT: “GO TO THE NEXT FORM” AND CONTINUE THE PROCESS UNTIL YOU SEE A MESSAGE SAYING “ALL FORMS HAVE BEEN COMPLETED”. SOME FORMS ARE LONGER THAN OTHERS. UPON COMPLETION OF FORMS, YOU WILL RECEIVE A CONFIRMATION E-MAIL.
8. THE DRUG TESTING CONSENT FORM ASKS FOR A “SPONSOR AND UIL EVENT”.
a. SPONSOR: PLEASE FILL IN THE NAME OF THE COACH FOR THE *FIRST* SPORT THEY ARE PLAYING. (i.e. SPORTS ARE FOOTBALL AND TRACK, SPONSOR NAME SHOULD BE “STAUGH”)
b. UIL EVENT: PLEASE FILL IN THE *FIRST* SPORT THE ATHLETE WILL PARTICIPATE IN (i.e. PARTICIPATING IN FOOTBALL AND TRACK, YOU WILL LIST FOOTBALL AS THE UIL EVENT)
9. TURN IN A COMPLETED PAPER PHYSICAL FROM YOUR DOCTOR TO THE ATHLETIC TRAINER! DO NOT TURN IN PHYSICALS TO A COACH, FRONT OFFICE SECRETARY, OR ANYONE ELSE. IT IS NOT THE FAULT OF THE ATHLETIC TRAINER IF PAPERWORK IS LOST DUE TO NOT BEING TURNED IN APPROPRIATELY.
